The Report of Performance Rating (P.O. Form 12) has been revised for enhanced usability. The Performance Evaluation System(PES), which was established in 1979, was out-of-date and therefore needed to be updated. The enhancements made to the Report of Performance Rating (P.O. Form 12) help bring the system into alignment with the District’s move to a performance-driven culture, where performance management is at the center of the day-to-day work performed by agencies.
Supervisors will be required to use the revised Report of Performance Rating (P.O. Form 12) when evaluating their employee’s performance during the April 1, 2007 - March 31, 2008 review period.
Below you will find a list of the revisions made to the Report of Performance Rating (P.O. Form 12), including:
- No more usage of “+”, “v” or “-“. Individual sub-factor ratings have been eliminated. A numeric rating will be assigned to each factor based on the parameters described under each item.
- No longer using the Performance Rating Worksheet. Instead, the overall rating will automatically calculate onto the Report of Performance Rating (P.O. Form 12).
- A narrative justification and written explanation will be written for each factor that is rated, as well as an overall summary of the employee’s performance.
- The highest rating category has been changed from “Outstanding” to “Role Model”.
