Mental Health Care Providers Can Pre-Register Online for the H1N1 Vaccine

 

Community based mental health providers who partner with the Department of Mental Health can register their sites now to receive the 2009 novel H1N1 influenza vaccine when it becomes available from the Department of Health and the Centers for Disease Control.  The Health Department has established online pre-registration for interested providers who want to administer the vaccine at their facilities during the H1Nl influenza vaccination campaign.  Providers who pre-register their sites must be approved but will receive vaccine planning information.

The Health Department expects that vaccine supplies will be available in mid-October, but could arrive later this month.  Before receiving the vaccine, a provider must complete a Provider Agreement that outlines the specific terms and conditions of vaccine use and accountability as defined by federal and state authorities. This agreement will be available shortly from the Centers for Disease Control and Prevention (CDC) and will be emailed or faxed to the H1N1 Point of Contact.

Providers with multiple locations must submit a separate pre-registration form for each vaccination site.  In addition, the form must include the name of the authorized prescriber (e.g., M.D., D.O.) under whose authority vaccinations will be administered.
