REMINDER NOTICE

Licensure renewal extended to August 30, 2012
 

If you have completed your renewal application and Criminal Background Check please disregard this notice. 

The purpose of this notice is to inform you that as of the above date, your license has not been renewed.  Please keep in mind that your license will not be renewed if:

(1) We have not received appropriate supporting documentation pertaining to section  “6” for which you may have answered “YES”; or

(2) You have not completed your Criminal Background Check Live Scan.

Once your license expires, you will have until October 30, 2012 to renew your license with an additional late fee of $85.00 and complete a CBC Live Scan. After that date, you must apply for licensure reinstatement.

Please note: Continuing education requirements must be completed by June 30, 2012

Allow at least 14 days after you have applied for renewal for your licensure status to be updated.
