Documents can be requested online, or you may download and complete the Ceremonial Document Request Form*. A list of available documents and samples may be viewed on the Ceremonial Services Unit page at the Office of the Secretary website. Final ceremonial documents can be mailed to the requestor or to the recipient or may be picked up at 1350 Pennsylvania Avenue, NW, Suite 419. Please allow a three-week turnaround time.
